Mandubular subcondyle fracture是車禍創傷中常見的骨折，subcondyle位置如下圖，為mandible condyle底下區域

對於此類骨折，一般而言治療選擇分為兩種：open treatment與closed treatment

• Open treatment = ORIF with maxillomandibular fixation
• Closed treatment = maxillomandibular fixation

 

簡單而言，open是將傷口打開，將subcondyle的部分先做ORIF後，術後配合(或不配合)將上下排牙齒固定幫助癒合，稱為maxillomandibular fixation，如下圖

通常open術式術後配合固定的時間較短，約2周

 

 

 

而closed treatment則是不開刀，直接進行maxillomandibular fixation，讓骨頭原地癒合，通常時間較長，約4周或以上

 

對於兩種處理方式孰優孰劣一直有其爭論，本次介紹一篇由Alexander R.等著作，發表於整形外科知名期刊plastic and reconstructive surgery的Mandibular Subcondylar Fracture: Improved Functional Outcomes in Selected Patients with Open treatment，討論在某些條件下，open與closed treatment的好壞

 

實驗設計

本實驗為一個single surgeon試驗，統計該名外科醫師於2013至2018年遇到的60名單側mandibular subcondylar fracture的病患。依據單雙月來選擇治療方式(如單月進行open；雙月進行closed)，總共將病患分成open組(33人)與closed組(27人)，其收案標準為在CT下超過15度displacement且超過3mm的mandibular height loss。兩組病患的情況如下

實驗結果，即治療後的預後，評估面向有術後症狀、下巴的偏移程度、malocclusion、嘴巴術前術後可張開的改善程度與使用FACE-Q量表評估的分數等，如下圖

可以發現在所有層面，open treatment的預後都是顯著優於closed treatment的

 

結論

在選定的病患中(單側mandibular subcondyle fracture，且CT下位移量超過15度；>3mm的mandibular height loss)中，使用open treatment──即ORIF+maxillomandiublar fixation (術中使用endoscopic assistance'

nerve monitoring)，相較於不進行手術的直接進行maxillomandiublar fixation的closed treatment，其在術後症狀等多項預後中皆是較優的
